We understand the benefits and challenges associated with these innovative payment models and support our practices in meeting the terms of their value-based care participation agreements established by payer contracts and/or government entities.

Administrative Support:
- Managing required data exchange
- Maintaining the current roster of applicable practitioners
- Facilitating provider agreement executions
- Submitting clinical episode categories and interventions (providers must load necessary data and documents in accordance with value-based contracting requirements)
- Communicating with providers on participation obligations and regularly sharing performance information

Management Of Clinical Pathways:
- Clinical pathway design for best practices based on specialty
- Monitoring provider performance, associated financial analysis and outcomes measures (reviewed on a quarterly and as needed basis), including deviations from evidence-based clinical pathways
- Tracking and managing patients throughout episodes of care (as outlined by the payer contracts and/or government entities), including utilizing care management software
- Patient education, intervention and collaboration with providers, staff, surgical facility personnel, post-acute care providers and/or ancillary providers in alignment with the MedVanta Care Redesign Plan and consistent with each individual’s scope of practice
